Players attending the National ODP Championships or the trip to the Czech Republic do not need to order a shirt on this form.  They will be provided one shirt as part of their event fee.  If players attending either of those events wish to purchase a second shirt, then they may use this form.  Please click here for an ODP shirt order form.
